New MHAAR Form

The MHAAR form is now broken up into five components to make it easier to download, to fill out and to process.  Each document will include questions on the name of the Facility, the Business Phone Number and the Street Address so we can coordinate all of the information in each form.

The Forms: 

MHAAR1Main.pdf  is the main content form where you include your contact information.  This form is the most important form.
MHAAR2Type.pdf will ask you about the Type of your facility.
MHAAR3other.pdf will ask you about the Age and Gender of Person’s seen, the Special Populations seen, the Paid Staff and Degrees, the Location of the Paid Staff and background checks and the Volunteers with their background checks.
MHAAR4services.pdf will ask you about the Services.
MHAAR5diagnosis.pdf will ask you about the Diagnosis.

Saving the Forms

You can save the forms on your computer while filling them out.  You do this by selecting File and then Save As:

select Save As in Adobe Reader from the File Menu

You can enlarge this image by clicking on it and then clicking off the image to restore the screen.

Please relabel the forms with the name of your organization and the corresponding labels for main, type, other, services or diagnosis at the end of your name,  this will make it easier for us to process your forms.

For instance if you are the NTSPP you would save each corresponding form as:

ntspp main
ntspp type
ntspp other
ntspp services
ntspp diagnosis

This makes it easier on our end to process the data collection in all of these forms giving an extensive comprehensive collection of information about each resource on our website.

And then email the forms back to us.
